Health Equity
We envision all residents of Calvert County having the ability to maximize their health through access to quality healthcare regardless of race, age, gender, sexual orientation, or socioeconomic status, therefore eliminating disparities in health outcomes.
The Health Equity team at Calvert County Health Department is helping to serve our community and decrease disparities by linking those in need to resources.
Highway to Health (H2H) is a mobile outreach van that is case management driven. The mobile outreach van travels throughout the county to provide screenings and linkage to services such as behavioral, medical, and social. This free program was set up by the Health Department to help clients get assistance with needs that they may have had trouble navigating in the past. Clients will be able to connect to health department services without traveling to the health department. The coordinatorsl also are able to link clients to other needed services outside of the health department.

Prevent T2 Program
Designed for those with prediabetes, this year-long lifestyle change program is divided into 16 weekly sessions followed by monthly sessions. The program goal is to reduce body weight by 7% and increase physical activity by 150 minutes per week. In person, Online and/or Distance learning workshops are available. Classes are no cost to you and are currently forming. Registration is required. Please visit our Prediabetes page to register or for more information.
Healthy Hearts Start at Home
The Healthy Heart Blood Pressure Self-Monitoring (BPSM) Program is an evidence-based four-month lifestyle intervention program to help people with hypertension learn how to monitor and control their high blood pressure with the support of peers.
