Adult Evaluation Reviews (AERS)

There are people who have health problems, which are acute, chronic and debilitating. These problems often are related to advanced age, physical or mental limitations, with many people institutionalized unnecessarily at high public cost. The lack of affordable personal care, the shortage of affordable assisted living facilities and the frustration experienced by supporting family and friends are all factors which contribute to neglect or premature institutionalization.

The Adult Evaluation Reviews Program provides assessments (an exploration of social, medical, financial, functional, and emotional status), develops care plans, and coordinates services on the population considered at risk of institutionalization. Services are coordinated with the psychiatrist and psychologist on all Pre Admission Screens and Resident Review nursing home clients. Our goal is to help people remain in the community or in the least restrictive living situation where they can function at the highest level of independence and personal well being.

Referrals

Referrals are accepted from any concerned individual by phone, fax, mail or in person. Referrals for pre-admission screening and resident reviews generally come from hospitals, nursing facilities, private physicians, or other health care providers.

Fee

There is no charge for evaluations.
