Death Certificates

Certified Copies of Death Certificates

If a death certificate has been registered at the Health Department by a funeral home, certified copies of death certificates will be issued at the Health Department within one month of the date of death.

To obtain a certified copy of a death certificate:

  • Appear in person at the Health Department
  • Complete the application
  • Provide proof of relationship to deceased.
  • $20.00 fee per certificate is required
    • Cash, checks, or money orders are accepted
    • Please make checks or money orders payable to: Calvert County Health Department
  • Hours: Monday - Friday: 8:00 a.m. - 4:00 p.m.

After one month, death certificates are only available by mail from the State of Maryland Division of Vital Records in Baltimore. The cost is $12.00. Checks or money orders should be made payable to the DIVISION OF VITAL RECORDS. Mail check or money order and completed application to the address on the form. Applications may be obtained from this health department.
